Entry Requirements 

1. Complete the Online Nominee Entry form. (below)

2. Documents supporting the nominee’s achievements and qualifications (education certificates, resume, letters of recommendation, customer letters, photographs, description of charitable efforts or FCAA volunteer work, marketing materials, company/community descriptions, photos, etc.). (MUST be transmitted electronically to taylor@fcaaonline.com) Please put the nomination category as the subject line.

4. Photograph of Nominee (MUST be transmitted electronically to taylor@fcaaonline.com)

5. Review Judging Rubric below to determine what criteria will be judged for each category.
6. Choose 'Registration Options' at the top of this page to pay your nomination fee. Invoicing is available.

 

All submissions, letter of nominations, and supporting documents MUST be received by Thursday, April 4th to be eligible to win.
